Catholic Charities Fort Worth - Bishop Kevin W. Vann Dental Clinic
The Catholic Charities Fort Worth (CCFW) Dental Clinic provides access to affordable, quality and safe dental care, dental health education and preventative services for economically disadvantaged individuals and families throughout our 28-county region. By appointment only. Potential patients must reside within one of the 28 counties served by the Diocese of FortWorth. The dental clinic does not restrict services in regard to religious affiliation, race, ethnicity, gender or disability. Patients qualifying for dental services must have a household income at or below 200% of the federal poverty guidelines.

Provides community mental health services. Use the main phone number to contact an office for an appointment or to ask for more information about eligibility and programs. The 24-hour toll free Crisis Hotline is (800) 621-8504. This crisis number is for people facing an emergency situation, such as having a homicidal or suicidal thoughts. If your call is an emergency, a crisis team may be sent out to help manage your situation. If necessary, emergency services may also be contacted to respond to your crisis.
VA North Texas Health Care System - Bridgeport Community Based Outpatient Clinic
VA North Texas Health Care System has a Community Based Outpatient Clinic (CBOC) program to provide health maintenance and preventive health care within 30 miles or 30 minutes of a veteran's home. Bridgeport CBOC serves the following counties: Wise, Jack, Clay, Archer, Baylor, Young, Montague and Throckmorton.

Participates in the Community Partner Program (CPP) through its Benefits Enrollment program. As a CPP site, staff will help people use YourTexasBenefits.com website to submit online applications, including uploading documents for the following programs: Food Stamp/SNAP, Medicaid, TANF and CHIP. Additionally provides outreach and individual application assistance to those interested in learning more about long-term care services and Medicare Savings programs (QMB and Extra Help). In addition, staff will help clients manage their benefit cases as well as assist with renewals and making online changes.

The SNAP food benefits (formerly known as Food Stamps) helps people with low incomes and resources buy the food they need for good health. Medicaid Programs provide medical benefits for eligible low income adults, children, and families. CHIP offers a health insurance program for children. Anyone convicted of a drug-related felony (possession, use or distribution of a controlled substance) after August 22, 1996, is not eligible for SNAP (Food Stamps) benefits.Hours: M-F, 8 am to 5 pm.
